Our Programs

Mobile Integrated Health (MIH)

The Covenant Health Partners MIH program delivers healthcare services in patients’ homes. Home visits can reveal unrecognized needs, helping connect patients to mental health, social, or financial support.

The MIH program is available to Lubbock County residents who have been diagnosed with Congestive Heart Failure (CHF), are receiving treatment at the Joe Arrington Cancer Center (JACC), or have recently received a kidney transplant. Patients in the program can request paramedic support at any time using the 24/7 Nurse on Demand phone line. Patient visits and status updates are communicated to the medical team via Epic or direct communication, which may reduce emergency room visits or hospital admissions.

Paramedics, specifically trained for this role, perform home assessments, offer disease specific education, review medications, and provide supportive care such as conducting physical exams, checking vital signs, monitoring weight, monitoring cardiac rhythms, administering IV therapy, giving medication, and collecting lab specimens.

Nurse Community Advice Line

The Nurse Community Advice Line provides residents with access to medical advice when they need it.

This advice is provided by licensed Registered Nurses who are specifically trained to answer questions related to routine health and medical issues. When necessary, nurses will schedule next-day appointments, provide home-care instructions or coordinate care via the nearest emergency room.

With a simple phone call, the Nurse Community Advice Line will evaluate and guide callers to the appropriate level of care.

Community Paramedicine Program

The Community Paramedicine Program uses specially trained paramedics to help patients manage acute and chronic medical conditions and assists with the implementation of care plans approved by the patient’s care team.

This program supports the Nurse Community Advice Line by performing hands-on assessments or escalation to an emergency medical facility where needed.

The Community Paramedicine Program helps give necessary care as close to home as possible and helps to prevent unnecessary hospitalizations.
